We’ve designed our referral process to be simple and efficient. Dentists can refer patients by completing our Dentist Referral Form, and patients can self-refer using our Patient Referral Form. Once a referral is submitted, our team will carefully assess it and contact the patient to arrange their initial consultation. Following a comprehensive assessment, we’ll provide a detailed treatment plan and keep the referring dentist updated every step of the way. Once treatment is complete, the patient will be referred back to their regular dentist for continued care.
